How do I place my membership on hold or cancel it?

Month-to Month Memberships may be cancelled so long as the Member provides at least 30 days notice prior to their next scheduled billing date. ALL Membership Packages automatically renew on a monthly basis unless written notice is received by FIT4MOM Flagstaff. In order to cancel a membership, the Member must fill out a cancel form https://forms.gle/DNpJ8o3vEaae3reF7 and email danarowson@fit4mom.com. Without email confirmation from a FIT4MOM representative and completion of the online Cancellation Form, the cancellation does not exist. It is up to Client to obtain this confirmation within 72 hours. We regret there are no exceptions. Any questions regarding cancellations must be emailed to danarowson@fit4mom.com No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Flagstaff franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged the normal registration fee. Instructors are not authorized to cancel memberships.

Members may place their membership on hold for reasons such as; pregnancy leave, long extended vacations, injuries, etc. All holds can be for a period of no less than 30 days and no longer than 4 months each year, not to exceed two different hold periods per calendar year. All Clients wishing to activate holds must fill out a hold form https://forms.gle/cKjaZZScebhtLFLS8 and email danarowson@fit4mom.com. Any questions regarding holds must be sent to Dana at danarowson@fit4mom.com. Once the end of the hold period has been reached, the Member’s original Membership contract will resume. Members must fill out the hold form 15 days prior to the desired hold start date. Any requests received after a Member’s billing date will be prorated accordingly- holds do not need to coincide with billing dates. There is a one time $20 charge to place a membership on hold. If a Member wishes to cancel during a hold period, the Member will still be responsible for full payment of their existing contractual agreement- hold periods DO NOT count towards the Member's monthly contractual obligation.
